第15回：開院１周年に寄せて

札幌柏葉会病院　−開院1周年に寄せて−

札幌柏葉会病院が開院して早くも1年が経過した。圧迫感のない病棟で患者さんや看護師さんたちの明るい声が聞こえてくるのを耳にすると、この病院が無事に船出できたことに安堵する。思えば2019年、コロナが世の中を変えるとは思いもよらなかった頃に新病院の計画を発表した。その後コロナが世界中を震撼させ、ロシアとウクライナの戦争が始まり、止まることを知らない円安が始まった。いずれも病院新築には向かい風ではあったが、都度多くのステークホルダーに相談をしながら竣工までに至ったことに感謝をしたい。新病院にはいろいろな構想が詰め込まれているが、端的に表現するとすれば医療安全から考えた医療最新機器の導入と、療養環境から考えた建築デザインや医療DXであろう。診断機器や手術機器を最新式のもので揃えたのは新しいものは医療安全に優れているということに集約される。優れた手術用ナビゲーションや術中の血管造影やCTなどは顕著な例である。病院の円形のデザインは病棟に死角がないという利点のほか、各々の病室から美しい札幌の景色と大きな空を見渡せるというメリットもある。病院では残念ながら亡くなる方も多い。人生で最後に見る景色がビルの壁で良いのか、という気持ちが常にあった。私なら空や山が見られる環境で死にたい。病院の建築費が高騰する中でこれをどのように周囲に説明するかが大変であった。少なくとも10年は他が追いつけない病院ができたと思っている。大切に使って欲しいと切に思う。今後この病院をどのように運営していくかは次の世代の役割である。2025年9月から中山若樹院長を中心とする新キャビネットが誕生した。昔“二番じゃダメなんですか？“と発言した政治家がいたが、一番になろうとしないものは二番にもなれない。中山院長を中心に札幌柏葉会病院は“北海道一“質の高い脳神経疾患医療を提供することを目指してくれるものと確信している。

四半世紀以上前に私は米国の病院でクリニカルフェローとして働いていた。当時の米国は建国の精神に溢れ、圧倒的な豊かさによってインフラが構築されていた。日本人がいくら勤勉であろうが、いくら手先が器用であろうが、凌駕できないインフラの差に打ちのめされた。多くの日本の病院が30年経った今でも劣悪な療養環境、労働環境で苦しんでいる。我々はそこを抜け出た数少ないチームである。その誇りを胸に北海道の脳神経疾患医療を牽引して欲しい。

最後に改めて今まで支えていただいたステークホルダーの皆様に感謝申し上げるとともに引き続き今後のご指導・ご鞭撻を心よりお願い申し上げたい。

Sapporo Hakuyokai Hospital — On Our First Anniversary —

It has already been a year since Sapporo Hakuyokai Hospital opened its doors. When I hear the cheerful voices of patients and nurses echoing through the spacious wards, I feel a sense of relief that we have embarked on a new journey. Looking back, we announced plans for the new hospital in 2019—at a time when we never could have imagined that COVID-19 would change the world. After that, the COVID-19 pandemic shocked the world, the war between Russia and Ukraine has begun, and the relentless slide of the yen has begun. Although they were all headwinds for the construction of the new hospital, I would like to express my gratitude that we were able to bring them to completion through repeated consultations with numerous stakeholders each time. The new hospital is packed with various concepts, but to put it simply, they can be summarized as the introduction of state-of-the-art medical equipment designed for patient’s safety, and architectural design and digital transformation (DX) in healthcare focused on the patient care environment. We decided to introduce the latest diagnostic and surgical equipment because of newer equipment is safer for the patient. Outstanding examples include advanced surgical navigation systems, intraoperative angiography, and CT scanning. The circular design of hospital not only ensures there are no blind area in the wards but also offers the benefit of allowing patients in every room to gaze out at the beautiful scenery of Sapporo and the vast sky. Unfortunately, many people pass away in hospitals. I always wondered if the last view of one’s life should not be the wall of the next building. If it were me, I would like to die in an environment where I could see the sky and the mountains.

With hospital construction costs skyrocketing, it has been difficult to explain these to stakeholders around us. I believe no other hospitals cannot catch up us for at least 10 years. I sincerely hope we'll take good care of it. How this hospital is managed and operated forward is the role of the next generation. In September 2025, a new cabinet led by Hospital Director, Naoki Nakayama has started. There was once a politician who asked, “Isn’t being number two good enough?” But those who do not strive to be number one cannot even become number two. I am confident that, under the leadership of Director Nakayama, Sapporo Hakuyokai Hospital will provide the highest-quality (number one) care for neurological disorders in Hokkaido.

More than a quarter of a century ago, I have worked as a clinical fellow at a hospital in the United States. At that time, the U.S. was brimming with the spirit of its founding, and hospitals had been built on the foundation of overwhelming prosperity. No matter how diligent or dexterous the Japanese doctors might be, we could not compete with U.S. medical system. Even now, 30 years later, many Japanese hospitals still have problems with poor patient care and working environment. We are rare instances that have managed to break free from this situation. Therefore, we have to be the leading hospital in neurological field in Hokkaido.

Finally, I would like to once again express my gratitude to all our stakeholders who have supported us thus far, and I sincerely ask for your continued guidance and encouragement.
